If pppd hangs up for whatever reason (for example because of an LCP echo failure), pppd does not exit, preventing wvdial from re-establishing the connection.

I believe I fixed this issue some time ago, and a test I did just now with the current code doesn't show the problem. If you find that the issue still exists with the current upstream pppd code (not the Debian package), then please supply detailed instructions for replicating the problem.
Honestly, there was likely some sort of hardware bug. Maybe the modem isn't properly hanging up when a connection is lost? We've found that we have to actually unplug the USB modem and plug back in to recover from lost connectivity (which occurs after a few days of being connected). We've resorted to a USB hub with port power switching.
We're running Debian 7 -- PPP version 2.4.5-5.1+deb7u2
