I would like to be able to generate an extent indicator rectangle within an inset map in composer, that shows the spatial extent of the main map like the attached example.

That's already possible - look in the map item properties under Overview. If you still have difficulties, please ask for help on the mailing list or at http://gis.stackexchange.com/
