Each list page on the UI now has a help button. Clicking this button displays a popup with description what the page represents. JIRA: PDC-714

The bootstrap modal dialog markup is quite verbose and duplicate in many places. It is replaced with template tag that loads it from a separate file and fills in the contents. The button that opens the dialog is moved into a separate template.
