Clear the patron info on top of checkin form when quiting it

Describe the issue
In the checkin form, I do a checkin for patron XYZ, who is displayed on top left of the form. I quit the page, and come back to it, ie to page /professional/circulation/checkout: the form is empty but patron XYZ is still displayed on top of it (see screenshot).

To Reproduce

1. Open the checkin form
2. Do a checkin
3. See the patron info on top of the form
4. Open another page, like the vendors page
5. Reopen the checkin form
6. See the patron info on top of the empty form

Expected behavior
The patron info is not displayed in this case.

Software version
ils.test.rero.ch (version 0.6.1)
ilspilot (phase 1 of tests)
