Failgrade not working in production environment

Hi. I am using moodle 3.9.5 (20210324) on both production and dev enviroments. Failgrade is working fine in dev env. restricting extra attempts on quiz, but this is not happening on prod env.

I checked on admin page and the plugin is correctly instaled on both envs.

I use docker containers, so I know both envs. are quite similar. Is there any configuration I should do to make failgrade to work? Or is there any other checks I should do?

Are you issue solved? It's quite a simple plugin, there is no extra configuration needed!
