Prevent duplicate diagnostics for same file in multiple projects #75

Sometimes the same exact file is referenced in multiple projects, which means when there are diagnostics, the diagnostics show up once for each project.

We should remove duplicates based on:

  1. file path
  2. diagnostic code
  3. the exact location of the diagnostic.
