Random Fatal Errors since 3 weeks now - need to deliver my survey as soon as possible

Hi,

I am trying to test a survey using the csv file uploaded here: https://raw.githubusercontent.com/PVatsBeer/BeerConjoint/master/survey/choice_questions.csv

When I try to test the survey sometimes it runs but it doesn't show the values in calories and alcohol content fields. Most of the times the workshop.formr.org website gives me fatal error.

I have understood that the error is coming from the read_csv line in the survey excel that I have created. I am unable to understand what else can I do because I have treid doing multiple things such as change the characters to avoid using "-" in the csv file to using "to" to show range of numbers. Still I receive fatal error. It's has been 3 weeks now and I am unabel to progress. Please help!
