Sitemap.xml not regenerated #139

sitemap.xml is generated once at startup, and then not regenerated as changes are made. As far as I can tell this is because sitemapUpToDate is not set to false when pages are added or removed, so the thread_SiteMap goroutine generates it once and then does nothing!
