Make all errors log from Processes or Tasks, and include process/task name in output

RIght now there seems to be some places where errors happen deep inside components (e.g. file operations in IPs and similar), where there is not much context information such as the current process name. This hinders effective debugging.

We should look at ways to try to have these errors be returned up to a place with more context, so that the error messages can include such info.
