Data refresh of consent and bulk data load is identified several issues in initial approach

Remove user/fhir_patient_id correlation table and any foreign key (not needed)
create Admin role and Admin user account
create post Bulk Data load user provisioning/mapping view to leap accounts for admin role only
allow Admin to create new account, map to fhir patient resource, persist, modify etc
demonstrate post bulk load user provisioning, and new account user provisioning as acceptance criteria.
