自定义 markdown-it 扩展不支持？ #250

不知道是我没找到入口还是确实是存在这个问题，我在使用这个库的时候需要对markdown-it 进行扩展，但是我没找到支持自定义markdown-it 插件的功能。

建议：

Engine的初始化参数里暴露一个markdown-it 配置项使得可以做到自定义（像vue-press）
或者吧现在的md对象暴露出来，去掉私有
再或者提供一个extendMarkdownIt的方法，使得访问内部md对象或者直接吧参数传递到md.use上
