不同形式的页脚问题

根据我们所里的要求，页脚需要统一改成这样。
请问有何办法实现？

老师，这个新的版本的上下页眉的线条都发生了变化。尤其是上叶眉，由两条变成了一条，下页眉的线也变错了。整体不太好看。应该如何调整一下？
