fix db:* init and migration features

Resolve rails rake db:* functionality for -dev to -test init (and migrations).

The -dev system has been defined 'by hand' and this needs to be integrated correctly into rails rake db:* functionality.

To be tested on the -test db, when firewall rules are modified for the new db server.

The db user init must allocate the correct permissions for inserting data.
