Show common values on demographic form

While a user is adding their demographics, show common values for each key next to that key's form area. Clicking a common value should add that value to the the relevant key's field--basically simulating as if the user had (for example) themselves clicked 'Add Race' and typed in whatever the common value was. If a common value is typed in manually, its style should change in the list of common values.
