W Categories has disappeared

Hello, I need to have the dropdown categories widget in 3 places on this page:http://sandbox.adhocbands.com/
The 3 locations are:
-at the bottom of the page - the links attach to category pages only if the dropdown is the only instance on the page

• I also added the dropdown categories to the actual page.php template - you'll see this at the top of the page under Find Your Music Here! This one works consistently (but with a delay), probably because it is the first instance called.
  -I also have the same category dropdown in the middle of the page builder template. Even if I remove the dropdown I coded in the page template itself, both of the dropdown lists don't work. They don't work if there is more than one instance on the page.

The theme author doesn't appear to provide support so I thought I would give it a shot here but I'm not sure where I am exactly. GitHub. Hmmm. SOS if anyone knows anything about any of this. Thank you!
