The auto generated TOC in .xhtml has no hierarchal structure.
ie: no indentation for different levels of headings.
ie: bookmark is flatten.
However, if I change the file suffix from .xhtml to .html, and open it up, the TOC (bookmark) works fine.

Is .xhtml not supported?

(This is fixed in newer release v4.13.3, currently newest is in https://www.npmjs.com/package/tocbot/v/4.18.2 )
