ensure solvers work with pathos and pyina maps

Instead of relying on some scattered examples, a test suite should be built.

Should test use of maps where solver uses penalties, constraints, and also caching. Should also test where solver has nested parallel maps (and ensure the parallel maps not only work, but also shutdown correctly -- see pathos ticket #33).

Should also include Mpi(nodes, source=True) and other non-default options. The use of source=True in the Mpi map is known to fail (see http://trac.mystic.cacr.caltech.edu/project/mystic/ticket/255).
