Since at least android 12, the per-app background restriction setting has 3 options: Unrestricted, Optimized, and Restricted. The default is optimized, which does partially restrict usage. The page currently suggests that background restrictions can only be enabled explicitly by the user (even if accidentally), which is no longer true.

Additionally, I've noticed that occasionally apps can be killed even if set to Unrestricted, especially if the app is not often interacted with directly. I believe "Adaptive battery" is the cause. This setting gives the system free reign to kill whatever it wants whenever it wants completely ignoring the background restriction settings. This needs to be massively emphasized on the page.

Adaptive battery is only briefly mentioned, and there are no instructions on the page to disable it. In Android 14, the adaptive battery setting was moved into the "Battery Saver" page. In Android 12-13 it was on the same page as adaptive charging.
