The ball is weirdly placed on the hand, sometimes above the hand, sometimes below and sometimes 'in' the hand. It even changes position over the hand with a jerky motion. Not sure if this is because of inaccurate tracking in the lab.

Apparently, the fixed joint in Unity is not really 'fixed'. This is a know issue. To overcome this problem, the ball is now being added as a child to the hand and is made isKinematic whenever it is held in hand. Those changes are reverted as soon as the ball is reverted.
