Useless documentLink responses for CSS classes referenced in the template

Volar provides textDocument/documentLink responses when hovering CSS classes in the <template>.

The problem with those is that the link only references the file path (which is likely always the same file path as the one that has the reference) and doesn't include the reference to the line:column. So clicking that link does nothing.

One way to address that is to include hash fragment with row:column reference so that clicking the link goes to specific location in the file.
