Squirrel Hill Health Center said late Friday that it had discovered and dealt with a potential data privacy breach in a database of some former and current patients.

The Pittsburgh-based health center said it had been alerted in early February to unauthorized activity on its computer network involving malware between Jan. 28 and Feb. 4. It began an investigation immediately with outside computer experts and found information that may have been exposed could have included names, addresses, dates of birth, Social Security numbers and diagnostic codes for what it said was a small number of people.

“The investigation determined that the malware was introduced into the system by an unauthorized actor and that certain files within Squirrel Hill’s systems were impacted,” Squirrel Hill Health System said in a statement. It has since secured the system, reviewing policies and procedures, and notified people who may have been affected, as well as state and federal authorities.

“We have no evidence any information was subject to actual or attempted misuse,” Squirrel Hill Health Center said in the statement. But it also advised people who think might have been affected to check bank and Explanation of Benefit forms and notify their banks if they see unauthorized or suspicious activity.
