A similar glyph to that used for the Ferneyhough quarter tone up and down is needed for Just Intonation symbols, to modify notes that have no other accidentals. A simple arrow up, arrow down, and extensions (two arrows up, down) as in the existing Extended H-E would be fine. Perhaps the base of the arrow should have a 'leg' or some other graphical element that would help in spacing such accidentals.

The Extended Helmholtz-Ellis Just Intonation accidental system is the work of Marc Sabat and it's hence not within my power as the maintainer of the SMuFL specification to determine whether or not such symbols should be added to this system.
In general I don't believe this request meets the (admittedly informal) criteria for inclusion into the SMuFL standard on its face, because you are asking for the creation of symbols that are not yet in use and therefore cannot, by definition, be considered standardised. SMuFL is of necessity a trailing indicator of the notations being used in contemporary art music: if we were to encode every novel symbol used by every composer in every new work, there would be little value to its role as a standard.
If there are existing symbols that are widely used to indicate raising or lowering a notehead by one or more commas, then those should be the ones that are encoded in SMuFL. Otherwise, unless Marc Sabat decides that these would be a useful inclusion to HEJI notation, I cannot take any further action on this request.
