Merge CR changes into master & update to track changes vs CR

We're entering a new phase on SVG 2!

With CR publication, we're going to need to be much more careful about any future corrections or changes to the editor's draft.

• Every change, even editorial changes or obvious corrections, will need to be recorded in a new section in the changes appendix.
• Any normative changes will need formal resolutions, as these could affect transition status.

Some basic cleanup needs to happen first:

• All the corrections and clean-up from the svg2-august-cr branch needs to be merged into master, except then the status needs to be flipped back to Editor's Draft.
• A new "Changes since CR version of 15 September 2016" section needs to be set up in the changes appendix.

I suggest moving to a PR based workflow so we are reviewing things before they go into the spec rather than what we do now where we hopefully highlight the changed text and maybe someone will look at it.
