TOC page is not generated in 0.12.5 nor in 0.12.6. I get no cover in 0.12.6

I'm using Windows 10.

I get the TOC.xml file in 0.12.5 and in 0.12.6 but I get no TOC on the pdf.

In 0.12.5 I can include a cover without any problem.
In 0.12.6 I can't include the same cover:
