Add discipline information to CodeFlag.xml

Summary and purpose

Since currently CodeFlag.xml does not have discipline information, for CodeTables 4.1 and 4.2, there are multiple elements associated with a single CodeFlag value, making it difficult to use them.
Adding discipline information to each GRIB2_CodeFlag_en element can resolve this issue.

Action proposed

• Extract SubTitle_en columns from CSV files and insert it as a SubTitle_en subelement to each GRIB2_CodeFlag_en element in CodeFlag.xml.
• Extract discipline information from SubTitle_en columns of CSV files and insert it as a Discipline subelement to each GRIB2_CodeFlag_en element in CodeFlag.xml.

The former would be easier although the latter would be more useful.
