This issue includes:

Hostname, ipv4, ipv6 tags are not supported. Related grcp using with skywalking-toolkit-opentracing #169
Inject and extract fail when using binary-map. Related Potential bug for OpenTracing activation, when format is "Format.Builtin.BINARY" #167
Peer.port is type of short, in some version of ot. Done in prev commits.
Refactor codes about inject and extract when using text-map. Potential bug for OpenTracing activation, when format is "Format.Builtin.BINARY" #167
Change context head-name to sw3. Lower case and shorter are better.
