Acceptance criteria:

• Form with following input fields:
  • Age
  • Gender
  • Postcode (patient’s home or hospital?)
  • Type of cancer
  • ECOG status
  • Gleason level
  • Disesase within prostate (a dropdown where they can select “focal disease”)
  • Disease outside prostate (a dropdown where they can select “advanced disease” and/or “metastatic disease”)
• Option to add another patient which will load another one of these forms below
• Find Clinical Trials button
