Various important updates:

Update kernel patch (4.19.257)
Update musl (1.2.3)
Make musl code easier to maintain (Preserve some if statements)
Use x86_64 versions of fabs and sqrt (less LOC and faster?)
Re-bootstrap on Debian Bullseye
Lots of documentation improvements
Fix musl build on modern systems (Disable some errors, still more paranoid than upstream musl)
Backport CONFIG_DEBUG_FORCE_FUNCTION_ALIGN_64B

Tested with LinuxCNC 2.9.0~pre0 (master branch)

This will be the last release of the RTAI 4 branch.
The 3.10 and 3.18 kernels have reached EOL, and the
3.16.53 kernel has introduced assembly changes that are
too complicated for me to work around.

All future development will now be focused on RTAI 5,
with performance improvements and clean-ups as always.
