app/views/submissions/_submission.html.erb

app/views/submission_notifier/form_submission.text.html.erb
app/views/submission_notifier/form_submission.text.plain.erb

app/helpers/forms_helper.rb
app/helpers/submissions_helper.rb

db/migrate/CURRENT_DATE_create_forms.rb

This will add to config/routes.rb
