fix HLT rate monitoring for CMSSW 9.2.x

• drop obsolete TriggerJSONMonitoring
• rewrite HLTriggerJSONMonitoring and L1TriggerJSONMonitoring to properly handle multiple framework streams
• check for conflicting values in the prescale column index
