Ranger: CUDA-enabled range helpers for C++ range-based for loops

Facilities for generating simple index ranges for C++ range-based for loops. Includes support for CUDA grid-stride ranges.

Examples:

// generate values from 0 to N
for (auto i : range(N)) {
