push_max_channel_subscribers readme stuff

some code cleanup: renamed ngx_http_push_shm_shpool to ngx_http_push_shpool, cleaned up some pointer typecasts. nothing major or logic-changey.

fix: publisher got a 201 Created response even if the channel had no subscribers at the time (should be 202 Accepted)

subscriber_sentinel should be reinitialized, not nulled
