Windows: Redesigned Effects window by moving preset/current preset to…

… the top of the window instead of aligned to the right. Replaced all label occurances of 'Effects' by 'Equalizer', and removed 'Equalizer (18-bands)' panel header since it becomes redundant.

Related to issue #422.
