[BUG] Same warning shows twice

Describe the bug
There is same warning showing twice when I open the code file with activated verible language server.
I guess it's related to how vs code processes the scope of plugin, i.e., vs code executes the plugin once the folder is opened, and then executes again when user opens the file.

• OS: Windows 10 21H2
• VS Code version: 1.82.8
• Extension version: latest
