Do we want to create an eligibility checklist?

Potential checklist:
a) did you work in this state in the past twelve months?
b) Are you:

Giving birth to a baby yourself?
Caring for or Bonding with a baby?
Taking care of yourself?
Taking care of a family member who is sick?
Taking care of a friend?
c) Roughly how much did you make?
