Comment reply to entries

As a user, I want to be able to interact with a Liveblog publisher. Since Liveblog entries are stored as comments, we can store replies to each entry as a nested comment and maybe use core's reply-to-comments to provide an inline reply experience.

The focus should still be on the Liveblog entries, but users should be able to dive into other viewer comments if they like.

I suppose that visitors should be restricted only adding comment replies to liveblog comments, or to other liveblog comment replies. In other words, should users be prevented from adding top-level comments?

Top-level comments should stay functional.

I think we may need to introduce the ability to enable/disable commenting on Liveblog entries, though (separate from the top-level comments).

@keoshi can help with mocks on how this might look/work, if needed.
