Move ingress yaml to manifest folder when azd cli is fixed #15

Issue Azure/azure-dev#2390 required ingress yaml to be deployed via hook script. Move to manifests for auto deployment once it is fixed.

Fixed and tested Azure/azure-dev#2405 with branch cq-fix-ingress. Will submit PR once azd fix is available publicly.
