Upon clicking, the automatic email will be sent to user who hasn't done the survey for the last seven days.
username: According to UTD login username, three English letters (case insensitive) followed by six digits will be allowed.
password: It requires one digit, one uppercase letter and one lowercase letter. The system doesn't support account creation.
Notice that the function only exist when user has already done the survey within the last seven days.
Do it by simply entering his or her NetID.
Fill in the rest of the survey. After clicking submit, the following window will pop out.
