Create and attach disk at VM provisioning time and delete the disk from the cloud backend when VM gets terminated.

I needed to upgrade https://github.com/grnet/kamaki to latest 0.13rc4 but there were side-effects:

This is currently blocked by grnet/kamaki#73 and also affected by grnet/kamaki#72

Do not return null for volatile disk. Zero means no disk anyway.

I have temporarily downgraded kamaki to 0.13rc3 which does not have this issue.
