Since the modifiers are an array within the negotiated price object data and the organization of the data is identifying unique pricing for a given service/provider, if the service with any of those modifiers result in the same price, same service code, same type, etc. then I feel the interpretation is any of those modifier values would be included in the billing_code_modifier array for the negotiated price object, so by default it seems to be an 'OR' situation. |

It would be helpful if the semantics were clear as to the meaning of an array with multiple billing_code_modifiers in the negotiated price object. Is that meant as an AND of the modifiers (and will only match a claim line with all of those modifiers) or is the correct interpretation of multiple modifiers an OR?
It seems like different payers are using this field in different ways and it would be very useful if the intention was clear in the spec.
The other option would be to change the field to allow it to be used either an and AND or an OR, but indicate that in within the array.
