A timer is started for each request put on the device's queue (see blk_start_request->blk_add_timer). If the request does not complete before the timer expires, the timer handler (blk_rq_timed_out_timer) will mark the request complete atomically: static inline int blk_mark_rq_complete(struct request *rq) { return test_and_set_bit(REQ_ATOM_COMPLETE, &rq->atomic_flags); } and then call blk_rq_timed_out. The latter function will call scsi_times_out, which will return one of BLK_EH_HANDLED, BLK_EH_RESET_TIMER or BLK_EH_NOT_HANDLED. If BLK_EH_RESET_TIMER is returned, blk_clear_rq_complete is called, and blk_add_timer is again called to simply wait longer for the request to complete. Now, if the request happens to complete while this is going on, what happens? Given that we know the completion handler will bail if it finds the REQ_ATOM_COMPLETE bit set, we need to focus on the completion handler running after that bit is cleared. So, from the above paragraph, after the call to blk_clear_rq_complete. If the completion sets REQ_ATOM_COMPLETE before the BUG_ON in blk_add_timer, we go boom there (I haven't seen this in the cores). Next, if we get the completion before the call to list_add_tail, then the timer will eventually fire for an old req, which may either be freed or reallocated (there is evidence that this might be the case). Finally, if the completion comes in *after* the addition to the timeout list, I think it's harmless. The request will be removed from the timeout list, req_atom_complete will be set, and all will be well. This will only actually explain the coredumps *IF* the request structure was freed, reallocated *and* queued before the error handler thread had a chance to process it. That is possible, but it may make sense to keep digging for another race. I think that if this is what was happening, we would see other instances of this problem showing up as null pointer or garbage pointer dereferences, for example when the request structure was not re-used. It looks like we actually do run into that situation in other reports. This patch moves the BUG_ON(test_bit(REQ_ATOM_COMPLETE, &req->atomic_flags)); from blk_add_timer to the only caller that could trip over it (blk_start_request). It then inverts the calls to blk_clear_rq_complete and blk_add_timer in blk_rq_timed_out to address the race. I've boot tested this patch, but nothing more. There are cases when cryptlen can be zero in crypto_ccm_auth(): -encryptiom: input scatterlist length is zero (no plaintext) -decryption: input scatterlist contains only the mac plus the condition of having different source and destination buffers (or else scatterlist length = max(plaintext_len, ciphertext_len)). ACPI: Waking up from system sleep state S3 Reason for this is that during suspend the LVT offset for the IBS vector gets lost and needs to be reinialized while resuming. The offset is read from the IBSCTL msr. On family 10h the offset needs to be 1 as offset 0 is used for the MCE threshold interrupt, but firmware assings it for IBS to 0 too. The kernel needs to reprogram the vector. The msr is a readonly node msr, but a new value can be written via pci config space access. The reinitialization is implemented for family 10h in setup_ibs_ctl() which is forced during IBS setup. This patch fixes IBS setup after waking up from S3 by adding resume/supend hooks for the boot cpu which does the offset reinitialization. Marking it as stable to let distros pick up this fix. As part of normal operaions, the hrtimer subsystem frequently calls into the timekeeping code, creating a locking order of hrtimer locks -> timekeeping locks clock_was_set_delayed() was suppoed to allow us to avoid deadlocks between the timekeeping the hrtimer subsystem, so that we could notify the hrtimer subsytem the time had changed while holding the timekeeping locks. This was done by scheduling delayed work that would run later once we were out of the timekeeing code. But unfortunately the lock chains are complex enoguh that in scheduling delayed work, we end up eventually trying to grab an hrtimer lock. This works for the case here, where the do_adjtimex() was the deadlock trigger point. Unfortuantely, in update_wall_time() we still hold the jiffies lock, which would deadlock with the ipi triggered by clock_was_set(), preventing us from calling it even after we drop the timekeeping lock. So instead call clock_was_set_delayed() at that point. Sadly this cannot be done in nojournal mode as that function can call sync_dirty_buffer() which would deadlock. Luckily in nojournal mode the race is harmless (we only dirty already freed buffer) and thus for nojournal mode we leave the dirtying outside of the buffer lock. All entries of wq_numa_possible_cpumask is allocated by alloc_cpumask_var_node(). And these entries are used without initializing. So these entries have wrong value. When hot-adding and onlining CPU, wq_update_unbound_numa() is called. wq_update_unbound_numa() calls alloc_unbound_pwq(). And alloc_unbound_pwq() calls get_unbound_pool(). Fix it by skippping calling into policy draining if all the blkgs are already gone. Fix it by skippping calling into policy draining if all the blkgs are already gone. The heuristic was introduced by commit e62e384 ("memcg: prevent OOM with too many dirty pages") and it was applied only when may_enter_fs was specified. The code has been changed by c3b94f4 ("memcg: further prevent OOM with too many dirty pages") which has removed the __GFP_FS restriction with a reasoning that we do not get into the fs code. But this is not sufficient apparently because the fs doesn't necessarily submit pages marked PG_writeback for IO right away. ext4_bio_write_page calls io_submit_add_bh but that doesn't necessarily submit the bio. Instead it tries to map more pages into the bio and mpage_map_one_extent might trigger memcg charge which might end up waiting on a page which is marked PG_writeback but hasn't been submitted yet so we would end up waiting for something that never finishes. Fix this issue by replacing __GFP_IO by may_enter_fs check (for case 2) before we go to wait on the writeback. The page fault path, which is the only path that triggers memcg oom killer since 3.12, shouldn't require GFP_NOFS and so we shouldn't reintroduce the premature OOM killer issue which was originally addressed by the heuristic. As per David Chinner the xfs is doing similar thing since 2.6.15 already so ext4 is not the only affected filesystem. Moreover he notes: : For example: IO completion might require unwritten extent conversion : which executes filesystem transactions and GFP_NOFS allocations. The : writeback flag on the pages can not be cleared until unwritten : extent conversion completes. Hence memory reclaim cannot wait on : page writeback to complete in GFP_NOFS context because it is not : safe to do so, memcg reclaim or otherwise. The patch here fixes the race against freeary, by acquiring or waiting on the sem_undo_list lock as necessary (exit_sem can race with freeary, while freeary sets un->semid to -1 and removes the same sem_undo from list_proc or when it removes the last sem_undo). After the patch I'm unable to reproduce the problem using the test case [1]. My system keeps crashing with below message. vmstat_update() schedules a delayed work in current cpu and expects the work runs in the cpu. schedule_delayed_work() is expected to make delayed work run in local cpu. The problem is timer can be migrated with NO_HZ. __queue_work() queues work in timer handler, which could run in a different cpu other than where the delayed work is scheduled. The end result is the delayed work runs in different cpu. The patch makes __queue_delayed_work records local cpu earlier. Where the timer runs doesn't change where the work runs with the change. Turns out this error is because of missing ENDPROC for assembly code as suggested by Stephen Boyd <sboyd@codeaurora.org>. Let's fix the problem by adding ENDPROC in two places to sleep34xx.S. Let's also remove the now duplicate custom code for mode switching. This has been unnecessary since commit 6ebbf2ce437b ("ARM: convert all "mov.* pc, reg" to "bx reg" for ARMv6+"). And let's also remove the comments about local variables, they are now just confusing after the ENDPROC. It causes fatal corruption on the next chunk on the 3264-bytes size class, which is the last size class that is not huge. For example, if the requested size was exactly 3264 bytes, current zsmalloc allocates and returns a chunk from the size class of 3264 bytes, not 4096. User access to this chunk may overwrite head of the next adjacent chunk. While accessing cur_policy during executing events CPUFREQ_GOV_START, CPUFREQ_GOV_STOP, CPUFREQ_GOV_LIMITS, same mutex lock is not taken, dbs_data->mutex, which leads to race and data corruption while running continious suspend resume test. Fix a short sprintf buffer in proc_keys_show(). If the gcc stack protector is turned on, this can cause a panic due to stack corruption. The problem is that xbuf[] is not big enough to hold a 64-bit timeout rendered as weeks: (gdb) p 0xffffffffffffffffULL/(60*60*24*7) $2 = 30500568904943 That's 14 chars plus NUL, not 11 chars plus NUL. Expand the buffer to 16 chars. I think the unpatched code apparently works if the stack-protector is not enabled because on a 32-bit machine the buffer won't be overflowed and on a 64-bit machine there's a 64-bit aligned pointer at one side and an int that isn't checked again on the other side.
