Form Fillable PDF no longer appearing in Foundry VTT

Describe the bug
I've been testing a form-fillable PDF - adding fields and bringing the PDF into Simple Worldbuilding to verify the fields are entered correctly. It's been working great until it suddenly fails to load the fillable fields. I can see them in PDFescape, and in Adobe Acrobat Reader DC, but when uploaded the form fields are gone. I've cleared my temporary files on my computer, shut down and restarted Foundry VTT, and created a new world. Nothing changes the result.

Hi @DustyGold - thanks for the report. Could you do two things for me?

1. Hit F12 to open the development console. Now go through the process of opening the PDF that is failing to render form fields. If there is any red text in the "console" section of the window that appeared when you hit F12, please send it to me via screenshot or copy paste.
2. If you are legally able, please upload the PDF you are having troubles with here so I can examine it.
