Let users define apps, templated or from scratch

Today an admin can define new apps (db structure / ui etc).

Sometimes there is a need for users to define apps that they can control. An example is a survey system where a user should be able to define a new survey, including layout and database support.

Feature: Create Quick Form

• Title, Description
• App Type: Templated | From Scratch (Create the app from another app)
• Parent App (New instances of the form should have a parent instance id ?)
• Private / Org / Public
• List of inputs / queries
• Views (Create / Edit / List)
