add ISSUE_TEMPLATE: Bug Report Form

adding an issue template for reporting bugs, including the following inputs:

• Issue Description
• Environment Information (ParquetSharp Version, .NET Framework/SDK Version, OS)
• Steps To Reproduce
• Expected Behavior
• Screenshots (Optional)
• Additional Context (Optional)

Thanks @naskio. I don't think it's worth having a special section for screenshots though, given this isn't a graphical application. Possibly the "Additional Context" area could include screenshots if they are relevant, maybe the description for that area could be something like "Add any other context about the problem here, including log output or screenshots if applicable"

I agree with you. It seems more relevant for me to merge them.
I wonder if it makes sense to add a template issue for feature requests. What do you think?
