Create a form to propose a new source/resource

Create a first draft of a form with Survey123 (https://survey123.arcgis.com/) that can be used to propose new sources or resources in order to be added to the page

A first draft of the form has been created but this needs to be improved by adding dynamic questions based on the answer to the territorial scope answer

I like the idea of using Survey123 and configuring it to publish a public web service only with resources approved by any of the administrators.

Some additional resources and comments about Survey123:

• There is a Survey123 JS library to inject a form as a widget (explained here)
• Form fields can be populated programmatically as explained here
  • We could even consider a list of political and environmental boundary layers, for example from the living atlas or any other source
• We can set rules to show/hide specific fields
• Map field can also register lines and polygons
