This PR adds an "Upgrading to v4" doc that lists the full set of breaking changes from v3 in a single place, to make it easier for folks to upgrade.
Note: this is different from the CHANGELOG, which does list all changes but only lists them by PR title. The new upgrading doc makes it so folks can easily upgrade without needing to read through the PR descriptions/discussions.
