Removed broken RegExp filtering
Because the replacements only work within the scope of each data buffer/string, it's unclear to the user why some apply once and some apply many times. This breaks semver but I don't think many folks use the package for --replace anyway. 🤷
Fixes a couple of README.md typos as well.
