This issue is related specifically to a template like a UTC. UTI, UTP, etc.
In the Manaan cutscene where you see a trio of Republic and Sith Soldiers arguing, players will be quick to notice that the lead Republic Soldier is a generic Republic Soldier whereas one of his buddies (man26_repsol.utc) is a Republic Officer with an old commoner's head.

I can foresee two of ways this odd one out can be fixed:
The Republic Officer is named "Republic Soldier" so either we can rename the character from "Republic Soldier" to "Republic Officer" or we can swap the appearance from "Republic_Officer_Mal_Old_White_02" to "Republic_Soldier_Mal_Old_White_02" so the odd Republic Officer looks less out of place.
An alternative fix to the 1st solution would be to swap the generic leader (man26_repsolar) with the odd Officer buddy (manm26_repsol.utc). That means the leader, man26_repsolar.utc, will either be a Republic Officer with the old white 02 generic head or a Republic Soldier with the old white 02 generic head whilst his unimportant buddy, man26_repsol.utc, will use the generic Republic Soldier appearance.
