feat: all.sas containing all macros in one file

Now all files can easily be included through one file, with the following commands:
- filename mc url "https://raw.githubusercontent.com/KatjaGlassConsulting/SMILE-SmartSASMacros/main/all.sas";
- %INCLUDE mc;
