Toolbox Icons Are Not Re-sized As Needed

Short Summary

Large, custom toolbox icons cause toolbox item names to disappear. Instead, toolbox icons that are too large should be re-sized appropriately.

Steps to Reproduce

Create a new command and set the icon to be the 25x25 variant and reload the toolbox and you will see a slightly large icon next to your command. Now change the icon to the 2000x2000 variant and reload the toolbox and now your command has gone. Why has it gone ? Well, because the image is so big that it obscures the text and there is nothing in that part of the image to give the hint that there is an image overlapping it.

Expected results

Toolbox items with custom icons re-sized appropriately such that the item name is shown.

Actual results

A blank toolbox entry that functions, but there's no idea what it actually is.
